Form Redesign

Some people think forms are boring, and I suppose they are: sparse, imageless, and often photocopied to near-illegibility. But I love creating forms for the same reason I love fanny packs: they are pure utility!

For this project, I re-designed an existing form to make it more legible, visually attractive, and more enjoyable to fill in. The original form is on the left, my re-design is on the right.
